Conditions Of Reporting

  1. The reported results are for the information of qualified medical practitioners only.
  2. The test results relate only to the parameter(s) tested.
  3. Results pertain to the specimen submitted.
  4. The results of all laboratory investigations are dependent on the quality of the sample as well as the assay procedures used.
  5. Partial reproduction of this report is illegal and not permitted.
  6. The reported results are for the referring doctor’s information only and should be interpreted with the patient’s other clinical information and other examination/diagnostics work up if any; keeping in mind the limitations of methodology and technology used for testing.
  7. Results relate only to the sample tested and the result of laboratory tests may vary from laboratory to laboratory and also in some parameters from time to time for the same patient; may be due to physiological variations, different methodology, different technology and its various limitations etc.
  8. In case of collected specimen, which are referred to TOUCHLABS from the referral centre; it is presumed that patients demographics are verified and confirmed at the point of generation of the said specimens and the result(s) related only to the sample received.
  9. A requested test might not be performed in case of one or more of the following reasons (a) sufficient quantity of specimen, (b) specimen quality unacceptable, (c) incorrect specimen type (d) incorrect identity of specimen and many other reasons not mentioned here.
  10. In above mentioned circumstances it is expected that a fresh specimen will be sent for purpose of the reporting on the same parameter.
  11. If the result(s) is / are alarming or unexpected, the patient is advised to contact the laboratory immediately for possible remedial advice.
  12. Tests are performed as per the test schedule given in the test listing.
  13. In unforeseen circumstances (non-availability of kits, instrumental breakdown & nature calamities) sites may not be reported as per schedule, however company will always ensure that the delay is minimized.
  14. All matters are subject to SURAT jurisdiction only.
